Getting a library card

In order to obtain a Linden Library Card, you must have identification and proof of residency.
A card is free to new patrons; however, if it is lost or stolen, there is a $3.00 replacement cost.
The card is valid for one year from the day of activation. Upon expiration, you will be asked to provide identification so that we may validate your card for another year

1) The applicant must have a current, valid Driver's License showing a Linden address

*OR*

2) The applicant presents one item from Column A *and* one item from Column B

A must show your Name and Address	B must show your Name
Utility bill (gas, phone, electric) Payroll stub Bank account statement Property tax bill Rental lease	Social Security Card Voter Registration Card Senior ID Current driver's license with an out-of-town address

1) The applicant is accompanied by a parent or guardian to verify residency as per the criteria listed above

*OR*

2) The applicant presents one item from Column A *and* one item from Column B

A must show your Name and Address	B must show your Name
Current report card Current progress report Current class schedule	School ID Card

1) You must be able to write your name

2) A parent or guardian must be present to verify residency as per the criteria listed above

3) A parent or guardian must sign both the library card application and the library card
